Yes, this does work. The transmitter doesn't need to be under your foot. Take the transmitter from your shoe and test it out: Squeeze the transmitter with your fingers routinely = little difference on your iPod Nike+ Nano. Now take the sensor in your hand and move it back and forth = Your Nike+ iPod Nano picks up the signal.
Do I have the Marware transmitter holder? No, I cut the finger off a $1 pair of ruberized cotton gloves and stuck my transmitter in that. Pinned the transmitter (in sock) to my shoe and tied it in with the laces. There was nothing nothing else available locally.

This Nike+ sensor holder is the best made one I have found. I have a cloth one with velcro, a pouch one big enough to hold a credit card & a small silicone one as well. The cloth one, you have to completely unlace to change from one shoe to another. The silicone one is difficult to remove the sensor to change it out. And the large pouch is just too big & cumbersome & looks hideous; plus the sensor moves around in it which gives inaccurate data. The Marware Sportsuit easily attaches, (no unlacing)& it looks sporty. It is also very secure in its design. It won't move around & give inaccurate readings with the Nike+ info. The price is reasonable.

Just as the other reviews helped me to purchase this product by saying it works great and is easy to implement...I may help you by saying it was just the same for me. I just put the sensor in the slot (made sure it was oriented to right side up) and then viola it works great, no need to be recalibrated (I did just in case because I don't really know the milage I run without the chip or online mapping, and it was working fine before the calibration as well it was still accurate post-calibration).
Overall, I am really happy because I was able to get the ascics that I like to run with and still use my nike+ to enhance my training. :)

I tried to just put the nike sensor in my laces but it kept falling out. I purchased this little attachment and now I dont loose it, so I guess that means it was perfect because my problem is completely solved. It fits nicely on the laces and can even be removed easily when you dont want it on there. i used to have problems with the same route coming up as different mileage, and now they are all the same. So i am not sure about the reviews that said it doesnt count the miles, that may be their sensor not this attachment.
If you are having trouble keeping your sensor on your shoes, definitely get this attachment.

Yeah, you need this thing. Unless you're only going to use your Nike+ sensor with Nike shoes.
This velcro strap fits under your shoe laces and allows you to slip the sensor into it easily, then folds over to hold the NIKE+ sensor securely. It looks a little nerdy; but, hey that's not all bad.
In order for the sensor to work properly it needs to be "Face Up". Because of the way the strap folds over, it's not immediately obvious which way to insert the sensor into the pouch. But, once you've done it a couple of times, it comes pretty naturally; hint, the image is wrong.
